Determine What Is Needed
Line General

There are many things to consider when planning for life after a stroke. Identifying needs and the resources available, along with creating an action plan to meet those needs are crucial. Many families have found it helpful to think about several different plans. This way it is possible to respond well to whatever happens. Social workers and therapists are the best sources of accurate, reliable information on options that fit the stroke survivor’s and family’s values and resources. Be sure to take a realistic inventory of both the strengths and weaknesses of the survivor and family.  Lastly, it is important to list and find answers to questions regarding the following areas in an open manner:

Learn About Available Resources

One of the most important things you can do is to educate yourself. Gather information in order to understand what has happened to you or your loved one, and what to do next. Learn about stroke, disabilities, and available treatment options. Find out what resources are available. This information can be found in a number of ways:

Find Resources
Line General

National Stroke Association has developed a resource directory of state and national resources to:

1) Help educate stroke survivors and families on the types of resources available, and
2) More easily find helpful state and national resources.

Take time to read through the resource descriptions. Think about which services would be helpful, then search the database for various organizations, agencies and programs that match your needs. Many of the listings in the database will help you locate more specific local resources. Other ways to find information and resources include:

  • Contact an area stroke program through a hospital or rehabilitation program
  • Attend a stroke group or club
  • Talk to other stroke survivors and caregivers
  • Contact local disability and brain injury organizations
  • Contact your local Area Agency on Aging
  • Visit a senior center for a copy of the local senior resource directory
