Allison Hooker is a Stroke Navigator, and provides education to 120 Stroke/TIA patients each month. She helped implement an array of programs for patients and family such as: Ask Me 3 & Teach Back (which focuses on Stroke patient literacy), A Time For You (for private visits with family to help alleviate fears as they assume their role as caregivers), Our Stroke Bridge Clinic (to transition patients from hospital to home), and an evening stroke group to be a continued resource to stroke survivors.
Allison follows-up with all stroke patients by phone to ensure their needs are met and questions answered following their discharge home. She is also an intricate part of our TIA Center who views every TIA as an opportunity to prevent a stroke.
Allison is the patient's connection to hope for the future.
She also provides stroke education at community events, and her passion for prevention is key to improving the health of the community one patient at a time. Allison was an honored member to the "Circle of Excellence", an elite group of peers nominated by peers for exceptional patient care. Allison is not only a gifted nurse but she truly is a remarkable woman, wife, and mother of two.
We are honored to have her here at Forsyth as a valuable team player and an essential contributor to each and every stroke patients recovery. That is why I nominate Allison Hooker as a "FACE OF STROKE".
